Guidelines for Faxed Permit
to Leave |
|
Parents who are unable to leave
work to pick up their student or who are having someone else pick up the
student, may fax a written request along with identification to the Career
Center at 994-6919.
Faxed request MUST include:
- Reason for
student’s dismissal and time student should leave.

Student’s name, grade and ID #.
- Name and ID #
of person(s) picking up your son or
daughter.

Parent’s signature, copy of current Driver License, plus
a telephone number where you can be reached for verification.
